COMPLETING THE 100 POINT IDENTFICATION CHECK.
 

As part of the E4 Healthcare registration process, you must provide us with a 100 point ID check.

To complete this part of the registration, you must upload CERTIFIED documents that satisfy the 100 point ID check.

To do this, you must:

• Photocopy relevant identification documents that you have (the below table shows what can be used). These documents can be photocopied on to seperate pages or on the same page if they fit
• Ensure that the identification documents you have photocopied add up to a 100 points or more (refer to below table)
• Take the photocopies AND your identification documents to an authorised officer so they can view them and certify the document

This list shows the point system with every type of identification item having a certain amount of points. You MUST include one item from the Primary Documents list and overall the points should be 100 or more.
Example - Passport (70) + Centrelink Card (40) = 110 points
