About the Role:
Our client is looking for a dedicated and experienced Clinical Care Coordinator to provide comfort and care to our residents and be a leader to the team.

This is a Full-Time position from Monday to Friday.

As a Clinical Care Coordinator, you will play a vital role in ensuring the seamless delivery of clinical care services within our facility. Reporting to the Facility Manager, you will provide leadership to the care team, coordinate clinical care activities, and support the implementation of person-centred care. This role is pivotal in maintaining our client’s commitment to quality care and compliance with aged care standards

The facility is one of our largest ones with just over 100 beds and needs strong people to help drive and nurture the home. It is an exciting opportunity to make a genuine impact and grow your career.

Key Responsibilities: 

  • Oversee the delivery of high-quality clinical care to residents.
  • Coordinate the assessment, planning, implementation, and evaluation of individualised care plans.
  • Provide leadership and mentor-ship to nursing and care staff.
  • Monitor and ensure compliance with aged care accreditation standards and relevant legislation.
  • Support and drive quality improvement initiatives within the facility.
  • Foster a culture of collaboration and continuous improvement within the team.
About You

To succeed in this role, you will bring:
  • Current AHPRA registration as a Registered Nurse.
  • Experience as a CCC or an experienced RN looking to step-up (at least 5 years as a RN).
  • Knowledge of ANAAC and strong documentation skills.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and staff.
  • Proven ability to manage and prioritise competing tasks in a dynamic environment.
  • A genuine passion for aged care and improving residents' quality of life.
Desirable Qualifications & Skills
  • A relevant qualification in aged care, health management, or a related field - eg. Nursing Degree and AHPRA Registered
  • Proof of Vaccination, WMQ Limited policy and Current Driver's License
  • Familiarity with current trends and challenges in the aged care sector.
