About the Role:
As the Clinical Care Coordinator, you will play a key role in overseeing and coordinating high-quality clinical care for our residents. You’ll lead a team of healthcare professionals to deliver exceptional, person-centered care, ensuring compliance with industry standards and fostering a positive and supportive environment.
This position requires weekend availability, with Tuesdays and Wednesdays off, offering a unique opportunity for work-life balance in a rewarding and dynamic setting.
Key Responsibilities:
- Clinical Leadership:
Oversee the clinical care team, ensuring best practices in care delivery and compliance with all relevant legislation and standards.
- Care Coordination:
Coordinate and monitor resident care plans, assessments, and clinical documentation to support holistic well-being.
- Team Empowerment:
Lead by example, mentoring and supporting staff to achieve professional growth and excellence in care.
- Resident & Family Engagement:
Build strong relationships with residents, families, and healthcare professionals to ensure continuity of care and a positive experience.
- Continuous Improvement:
Drive quality improvement initiatives to enhance care standards and operational efficiency.
About You:
To excel in this role, you will bring:
- Proven Clinical Experience:
At least 2 years of clinical leadership experience in residential aged care or a similar healthcare setting.
- Professional Qualifications:
Current AHPRA registration as a Registered Nurse.
- Leadership Skills:
Strong team leadership and mentoring abilities, with a focus on fostering a positive work culture.
- Clinical Expertise:
Sound knowledge of aged care standards, care planning, and compliance requirements.
- Effective Communication:
Excellent interpersonal skills to engage effectively with residents, families, and staff.
- Flexibility & Availability:
Willingness to work weekends, with scheduled days off on Tuesdays and Wednesdays.
Desirable Skills & Qualifications:
- Experience in dementia care and complex clinical needs.
- Relevant postgraduate qualifications in aged care, healthcare management, or leadership.
- Current driver’s license and proof of vaccination (as per industry standards).
